Welcome to our practice! Dr. Michael Casagrande and staff look forward to providing you with excellent care and getting to know you and your family.

Policies
As a patient in our practice, you have the following rights, as well as responsibilities that have been listed below. These rights and responsibilities are made with the purpose of ensuring that our practice continues to run smoothly, thus giving us the opportunity to continue to provide a superior level of patient care.

Patient Responsibilities

  • ​Be on time for your appointment. Notify our office if you are running late due to extenuating circumstances. If you will be more than 15 minutes late you will have to reschedule your appointment unless the schedule permits.
  • There is a No show fee of $40 for regular appointments, and $70 for Physicals. If you cannot make your appointment, we require 24 hours notice, or the fee will be charged. Notice may not be left with the answering service or voice mail. Give the name of the person you spoke to.
  • New patients: must arrive 20 minutes prior to your appointment so we can verify your insurance. Please have your paperwork filled out prior to appointment time (download and print your forms at the bottom of this page).
  • A photo of you will be taken to become a part of your medical record per government regulations.
  • You are required to provide current/updated demographic information, pharmacy number and insurance information per regulations.
  • Your insurance card and drivers license with you are required at every appointment that you have.
  • Co-payment co-insurance, deductible and/or past balance(s) are due at check-in.
  • Read our Financial Policies.
  • Please be respectful to our staff.
  • Referrals: If a referral is needed to a specialist let our office know before your appointment. We require a minimum of 48 hours prior to your appointment to provide a referral to your specialist.
  • Medication refills: Allow 48-72 hours for all controlled-substance prescription refills. For all medications, call your pharmacy and request an electronic refill on your medication, or request a refill through your FollowMyHealth Portal. Your refill will not be processed if you simply call our office. 
  • We do not provide care for any Affordable Care Act Health Plan, Worker’s Compensation, or car-related accidents. If you still wish to be seen, you will be required to be self-pay.


Patient Rights

  • Receive quality health care.

  • Expect that all communications and records pertaining to your health care will be treated as confidential.
  • Take part in wellness programs.
  • Receive assistance from your insurance company’s customer service for concerns and questions.
  • Read your rights in the Notice of Privacy Practice


Insurance Carriers
We welcome new patients to our practice and are pleased to announce that we accept most insurance carriers for your convenience. Please call our office and our staff will help you determine your patient benefits as estimated by your insurance carrier.
